Thank you for scheduling with Johnson Integrative Health. We can’t wait to see you. Please complete the quick steps below so your visit goes smoothly

  1. Create/Login to the Patient Portal to confirm your detail and see your appointment info.

  2. Complete your intake forms and consents (new patients) or update medications/allergies (returning patients)

  3. Upload your insurance card and ID (if using insurance). Cash/DPC patients can skip this step.

If you are using insurance please make sure to upload information to your portal.

Bring your insurance card and a government ID with you to the appointment.

DPC members will not be charged for any covered services.

RENEW360 Clients

If you booked a discovery call we will go over your overall health goals and review program options. If you are ready to move forward we will get consent and intake forms complete.

The hair analysis test kit will be shipped directly to you or you may make an appointment to come to the office for sample retrieval.

Test results take 7-10 days to come back. Once they are received we will call to get you scheduled for an appointment to review the results and go over next steps.

Telehealth Appointments

A link will be sent to your email before the appointment.

  • We are located in the Chase building. The office is in a law office that remains locked at all times. Please ring the door bell and someone will come get you.

  • Parking is free and open.

  • Arrival: Please arrive for appointment 10 minutes early to verify details. We are located in a law office that stays locked at all times. Please ring the doorbell when you arrive.

  • Bring: Photo ID and insurance card (if using insurance)

  • Cancellation/No-Show: If you need to reschedule, please do so at least 24 hours before your appointment. If you cancel within the 24 hour period or no show your appointment a $50 fee may be charged.

  • Billing: Insurance will be billed for eligible services; copay/coinsurance may apply and is due at the time of the visit. DPC covers primary care services under your membership. Non-covered services will be discussed in advanced.

  • Communication: Secured messages are available through the patient portal for non-urgent needs. DPC members may contact the provider at anytime through the phone number provided. For emergencies, please call 911.
