Primary care services include:

- 1 annual physical/wellness visit: includes any necessary labs as determine by your healthcare provider and yourself.

-Acute visits: For problems such as UTIs, simple injuries, or like abdominal pain.

-Chronic care Management: We will come up with a plan together on how best to manage and follow up on any chronic conditions.
