Thanks for scheduling with Johnson Integrative Health. We can’t wait to see you. Please complete the quick steps below so your visit goes smoothly

  1. Create/Login to the Patient Portal to confirm your detail and see your appointment info.

  2. Complete your intake forms and consents (new patients) or update medications/allergies (returning patients)

  3. Upload your insurance card and ID (if using insurance). Cash/DPC patients can skip this step.

Patient Portal

Primary care visits (Insurance or DPC)

Please complete the standard intake and medication list. If you are using insurance please make sure to upload information and bring your insurance card and a government ID with you to the appointment. DPC members will not be charged for any covered services.

Functional Medicine Visits

Please plan for a deeper assessment during your visit. Initial visits last around 90 minutes and follow up visits take around 60 minutes. Complete the functional medicine intake form. Please bring any recent labs studies to your visit or upload them in to patient portal.

Telehealth Appointments

A link will be sent to your email before the appointment. Join the visit from a quiet, private place with a good internet connection. Have your medication list and any logs (BP, glucose etc.) ready before the appointment

  • Parking: We are located in the Chase building. Parking is free and open.

  • Arrival: Please arrive for appointment 10 minutes early to verify details.

  • Bring: Photo ID and insurance card (if using insurance)

  • Cancellation/No-Show: If you need to reschedule, please do so at least 24 hours before your appointment. If you cancel within the 24 hour period or no show your appointment a $50 fee may be charged.

  • Billing: Insurance will be billed for eligible services; copay/coinsurance may apply and is due at the time of the visit. DPC covers primary care services under your membership. Non-covered services will be discussed in advanced.

  • Communication: Secured messages are available through the patient portal for non-urgent needs. DPC members may contact the provider at anytime through the phone number provided. For emergencies, please call 911.

  • Returning patients only need to update medications, allergies, and any changes since your last visit.

  • We currently accept Aetna, BCBSOK, HealthChoice, Medicare of Oklahoma and certain Medicare Advantage plans (Aetna and BCBS).
